Well, there are two things I can think of. First, although it is not mentioned anywhere, I believe your combat rating has to be over 60 also. Second, you can use the following really cool cheat code. I copied this work from another forum, so I take no credit for it's contents, but I did test it and it works. Hopefully you are playing on an emulator as this is actually a hex edit code.

I haven't confirmed each name yet, but the names seem to follow the same order as in Dirtyferret's 'Sailor List' FAQ. I was able to add both Iden Leis (41) and Khayr ad'Din (40) using that list as my guide. Just make sure to pay attention to the number Dirtyferret assigned to that sailor since he doesn't always list them in order.
Oh, and just to correct the code a little since you can have up to 30 mates (including the character you're playing)
7E41xxyy where xx is 60-7D (Although I wouldn't advise using 60 or 61. 60 used for the main character, 61 used for your NPC first mate in the story line. Probably shouldn't use 62 if you're playing as Catalina or Joao or 63 if you're playing as Joao either since they pick up extra mates in the beginning as part of the story) and yy is the sailor ID
In my case, I had seven mates including myself, so my code was 7E41684F. The # 8 means 8th mate and 4f is Philly's code. Sure enough, there he was on my list. If you can't enter PAR or hex edit codes, I believe there are mathematical conversions to other codes.
